The notice a landlord serves before filing for unpaid rent.
The month or period the rent was for.
In Rhode Island, the 5-day demand notice gives the tenant 5 days. Paying the full amount inside the period stops the eviction, by statute.
R.I. Gen. Laws §§ 34-18-35, 34-18-56(a): Sent once rent is 15 days in arrears, in the form of § 34-18-56(a); suit may be filed no earlier than the 6th day after mailing. Paying in full before suit cures, and paying arrears and costs at the hearing also cures unless the tenant had a demand notice in the prior six months.
Every field is optional; a blank one prints as a line to write on. The notice is in English, as it is served in the US.
