Fill in the eviction notice template Arizona landlords serve

The notice a landlord serves before filing for unpaid rent.

The month or period the rent was for.

In Arizona, the 5-day notice to pay or quit gives the tenant 5 days. Paying the full amount inside the period stops the eviction, by statute.

A.R.S. § 33-1368(B): Before a special detainer action is filed, tendering all past-due rent and a reasonable late fee reinstates the lease; after filing, reinstatement also requires costs and attorney fees, and after judgment it is the landlord's choice. A mailed notice counts as received five days after mailing.

Every field is optional; a blank one prints as a line to write on. The notice is in English, as it is served in the US.
